Protecting your privacy is important to us. We realize that you have an interest in how we may collect, use and share information about you. Collecting personal information from you is essential to our ability to manage and continue your Providence Plan membership. When you enroll in Providence Plan, we at Accela Marketing Group, L.L.C., collect from you nonpublic personal information including: your name, address, and billing information. As part of that process we may collect other information from our affiliates or others such as transaction information for the payment of Providence Plan memberships. We may share your nonpublic personal information with other organizations in which we have joint marketing agreements (with the exception of financial transactional information) or as permitted or required by law. Members or participating provider networks do not share any information regarding your health care or services received with Accela Marketing Group, L.L.C. Therefore, this information is not received by, held, maintained or shared with any third party by Accela Marketing Group, L.L.C. We maintain physical, electronic and procedural safeguards that comply with federal and state regulations to safeguard your nonpublic personal information from unauthorized use or improper access.

If you wish that your nonpublic personal information not be shared for marketing purposes, please send us notification of your request in writing to: Providence Plan Privacy Notification, 200 Crescent Court, Suite 60, Dallas, TX, 75201. Please include your name, address and Providence Plan ID number in your correspondence.
