Providence Plan is not insurance; it is a discount medical plan. There are no forms, no maximums, and no limits on the frequency of care. Procedures are discounted at the time of service.

To receive discounts on dental services, Providence Plan members must make an appointment with a participating dentist. Names of participating dentists near your home are printed on the back of the welcome letter and ID card carrier sent upon confirmation of membership.
